Intro Appointment

here's how your first meeting with us will go

Objectives

There are quite a few things that need to get done before you can continue with us. We've compacted them into a single meeting for your convenience.

You do not need to prepare to answer questions. All you need to do is be physically prepared.

Clerical

  1. Identity verification. We need one or preferably two pieces of government-issued identification to prove who you are, when you were born, and your residency. Photos of the documents and of you with the documents will be taken, and the details will be filled in a form. Photocopies or photos of documents are not accepted.
  2. Contract review. We will go over the contract that governs our relationship, including your right to terminate it at any time, and sign it.
  3. Consent to participate. We will read and sign the consent to participate in an adult scene form (SP-02). Though this scene will not be published, we will be taking photos, so we need to sign this form. Note that we will NOT sign the model release form (SP-03), which means we have no right to distribute anything created.
  4. Acknowledgement of obligation to inform of health conditions. We will read and sign a form that contains an acknowledgement that you are obligated to inform us of any health issues relevant to the nature of this work.
  5. Non-disclosure agreement. We will read and sign this form that binds you, our employee/contractor, and our company as a whole to not disclose any personally identifying information about you or the employee/contractor. You may opt out of this form.
  6. Review of the shoot inspection procedure. We will review the procedure that has maintained our zero-infection over the years. This is, in some form, the procedure you will follow before your scenes. We will not conduct this procedure for this appointment as it is costly and unnecessary for the intro, but we can demonstrate portions upon request.
  7. Verbal consent statements. We will read consent statements to permit the questions, measurements, photographs, and optional additional measurements.
